when you get a new keychain it represent the world you get it from so were do the keychains come?

thats not entirely true because are some keychains you get other ways like you get lady luck from a chest and you make ultima weapon

This is what is highly accepted and believed. Perhaps in BBS the actual idea will be confirmed. Seeing as they don't have keychains in the beginning, but it's possible they get some later on.
We have to remember that the keyblade you have equipped is what shows up in all the cutscenes, as far as we know the recent scans and what not just have them with their started keyblades just like Sora in all of his cutscenes before the game actually came out.

There could be alot of reasons.
IMO it's represents what you've been through. Such as how much you've accomplished and the condition of your heart.

The other keyblade weilders like Riku have accomplished alot,the darkness and such. So they dont just represent the world you've got them from. Because Riku's keyblade didn't have a keychain that represented Hollow Bastion.
