1. Yes. PSPs are region free. You can play any game, regardless of where it was purchased.
2. You can probably figure things out eventually with a bit of finagling.
3. Secret Episode, some new commands, the Rhythm Mixer command style, and the three new Mirage Arena missions featuring Monstro, Armor of the Master and No Heart.

And an oft-forgotten added aspect are unversed symbols dispersed randomly throughout the world. They open up these weird little mini-games when you approach them.

How would you go about doing that. may need the patch

Here are the instructions of what to do to your .iso

Auto-patcher by ANiMeX (v1.0.3)

Use an untouched ISO to use the auto-patcher.

How to use the auto-patcher: (with Prometheus v.5 included)

1. Download the auto-patcher here: MEGAUPLOAD - The leading online storage and file delivery service
2. Extract the file.
3. Run the file named "Kingdom Hearts BBS Final Mix.exe".
4. Click on the "1) Check MD5" button, then locate and choose the untouched ISO of KHBBS FM.
5. When your ISO passed the MD5 check (it means it's untouched), click on the second option named "2) Apply Ripkit".
6. After the patching is done, you can now transfer it to your memory stick and play

In terms of cutscenes, you wouldn't be confused in the slightest unless English isn't your first language :3 all cutscenes bar those in the Secret Ending have English audio tracks with Japanese subtitles.

Basically when a PSP uses an UMD, it's just a storage place for the .ISO file that's on the disc. ISO files are just disc images, not limited to being solely on PCs. Is your PSP hacked first of all, if not, you can't do this because a non-hacked PSP can't run .ISOs.

Aaaaand thats probably the point where you should be asking people on forums other from this one. I've played the Final Mix version on my friends hacked PSP with the english patch applied and honestly its not that different from the US version.

The important (read: canon) secret boss is included in the international version, the Mysterious Figure.

The secret episode is cool, and it's fun to fight some heartless on the PSP, but you certainly arent missing much, especially given that you can watch the entire final episode playthrough in HD on youtube.

The Unversed mini missions aren't that great, and the commands they yield are cool at first, but the novelty wears off pretty quickly.

The Rhythm mixer command style is cool and very powerful, but you won't unlock it until you master one of the mini games in Disney town. (This isn't actually that bad)

The addtional secret bosses are fun and quite a challenge, but not in terms of skilled gameplay (read: MF). Monstro is a case of spamming mines/surges and healing. Armour of the Master is a bit harder, and when he absorbs your command style he can OHKO you if you get unlucky. No heart is a pain in the ass, but only because of his rediculously high defense, HP and attack power. His attack pattern is pretty simple, but ifyou are underlevelled you are going to face hell.

Point is, you arent missing much by getting a legal copy of the international version. Plus you get to keep the shiny box and your soul can rest easy!

(this perpespective come from someone who did a critical playthrough of BBSFM, reaching level 45 max on all 3 characters(and still hasn't beaten no heart :-<))
