I find this interesting because of what Mickey says. "Why would I know him?". I have no idea what it means, but is the real Mickey also in the Realm of Sleep?

I'd say they said it at the same time, but at the start of it, they have Sora and Riku talking at the same time but on separate lines.
There's no way Terranort and Vanitas could have fused though, right?

I thought the exact same thing when I read it. Although, it could actually end up being a Riku version of Xion o:

Think about who had the final fights with each of them in BBS. That's my justification for why they appear simultaneously :3
