You have to go to TWTNW and you get it randomly when you use master form. After you get it the first time you can use it in any other world you want to.

thanks guys, but it seems like whatever form i use, final form WONT appear.. how many times average do you think it should take, because ive been doin it for like 2 hours now..

I got the final form the very first time i transformed after that scene so i got mine really quick but i cant tell u an exact number that it takes. Just make sure the scene at memory tower has taken place.
