According to NikkiMirai from Gamespot Forums, he says that this is the list of worlds for KH3D and the storyline. I'm calling it BS, but thought I'd share it. Don't attack the messenger.

*Sora and Riku see off Kairi and travels to Mysterious Tower.

-Mysterious Tower (Realm of Light)
*Yen Sid explains Mark of Mastery and Realm of Dreams.
*Riku and Sora sleeps.
*BBS flashback. Master Xehanort with Ventus in Destiny Islands.

-Traverse Town
*Sora meets Neku and the Dream Eaters.
*Sora fights against Nightmares and stablish a relationship with Spirits.
*Neku takes Sora around Traverse Town. Xehanort appears and broke his deal with Neku&Others. Sora felt sleep.
*Riku meets Shiki and the Dream Eaters.
*Riku fights against Nightmares and stablish a relationship with Spirits.
*Riku meets other TWEWY characters and fight against a Nightmare boss. Riku felt sleep.

-Mysterious Tower (Realm of Light)
*Lea meets Yen Sid. Lea sleep and travel to the Realm of Dreams.

-Hollow Bastion
*Lea wakes up.
*Sora arrives in Hollow Bastion. Xehanort confronts him.
*Riku arrives in Heartless Castle. Maleficent confronts him.
*Xehanort tries to kill Sora but Lea saves him. Sora and Lea fight Xehanort.
*Vanitas appears near Riku. Riku is confused because his darkness inside. Vanitas tries to bring him to the dark side.
*Xehanort dissappears. Sora and Lea tries to find Riku and they see him with Vanitas. Sora felts sleep and Lea tries to save them, but Vanitas hurts him.
*Riku fights Vanitas. Lea explains that Xehanort is trying to forge de X-blade.
*Riku and Sora say goodbye Lea.
*Mirage Arena is unlocked.

-Radiant Garden (Realm of Light)
*Flashback (before KH1). Xehanort summon his keyblade and release Ansem's apprentice hearts.

-Pizza Planet
*Mini-games world.

*Sora meets Joshua
*Yo appears. Neku and Sora fight him.
*Riku meets Beat and Rhyme.
*Xehanort appears and summon a Nightmare boss.

-Hollow Bastion 2nd visit
*Sora and Riku are transported to Hollow Bastion by darkness.
*Vanitas appears. Riku and Sora fights together against him.

*Lea meets Isa, but he's a mirage.

-La cité des Cloches 2nd visit
*Movie plot.
*Riku tries to fight his darkness inside and Heartless Ansem appears.
*Sora meets Xehanort in NotreDame. Sora fights Vanitas.

-Pranksters Paradise 2nd visit
*Riku wakes up inside Monstro. Another Riku appears, it's the manifest of the damage caused by Riku in KH1.

-Beauty Village 2nd visit
*sora wakes up and fights Gaston.

*Vanitas tries to defeat Sora to release Ventus' heart.
*Vanitas is defeated and Xehanort appears.
*Final boss: Xehanort at No Heart's armor.
*Ventus wakes up. The Realm of Dream is sealed and Sora, Riku and Lea appears at the Chamber of Waking.

-Destiny Islands
*The arrives to the Islands and meets Kairi and others.
*The sky turns grey and Xehanort (Terra's body) appears emerging of the water. Sora, Riku, Ventus, Lea, Kairi, Mickey, etc are ready to fight. Then, "Kingdom Hearts III" logo appears.

-Secret Ending

On the topic itself...I am skeptical, very skeptical on this since it makes no mention at all on Ansem the Wise's data which is said to be a focal point of DDD.
Not to mention Ventus already waking up and being completely healed...the rescue of the tormented ones is supposed to happen in KH III, not in KH DDD.
